There are two options for your purchase of our spay/neuter certificate

* Online payments are automatically charged a processing fee by Pay Pal
* We will be notified of your purchase via email and your certificate will be mailed to you within a few days, including the list of participating veterinarians. The certificate authorizes the veterinarian to bill HSLC directly for payment of the spay/neuter procedure only
Through the Mail

Required Items
- Check or Money Order made out to “HSLC” for the appropriate amount as indicated under Certificate Fees at the bottom of this page.
- Self-addressed, STAMPED, #10 (business) envelope (for certificate to be mailed back to you)
- List of the requested procedure as follows:
Spay (female) or Neuter (male)
Type of Animal (cat, dog, rabbit)
If female, is the animal in heat or suspected of pregnancy? (additional charges may apply)
If a dog, what does he/she weigh? (see Certificate Fees according to weight)
4. Include: Name, Address, Telephone, and Email address so we may contact you with any questions
You will receive your certificate via return mail, including the list of participating veterinarians. The certificate authorizes the veterinarian to bill HSLC directly for payment of the spay/neuter procedure only
You must present the certificate to the veterinarian to receive the reduced fee.

Male - $50
*Female - $100
Dogs:
Male (40 lbs or less) - $70
Male (41-80lbs) - $85
Male - (Over 80lbs) - $100
*Female - (40lbs or less) - $100
*Female - (41-80lbs) - $150
*Female - (Over 80lbs) - $200
